{"id":3872,"date":"2020-04-03T12:06:56","date_gmt":"2020-04-03T04:06:56","guid":{"rendered":"https:\/\/pnl-law.com\/blog\/?p=3872"},"modified":"2020-05-23T23:15:29","modified_gmt":"2020-05-23T15:15:29","slug":"documentary-requirements-to-avail-of-government-social-amelioration-programs","status":"publish","type":"post","link":"https:\/\/pnl-law.com\/blog\/documentary-requirements-to-avail-of-government-social-amelioration-programs\/","title":{"rendered":"Documentary Requirements to Avail of Government Social Amelioration Programs for COVID-19"},"content":{"rendered":"\n<p>Unless otherwise specifically stated in Joint Memorandum Circular No. 1, the following are the specific documentary requirements to avail of the social amelioration programs (see <a href=\"https:\/\/pnl-law.com\/blog\/social-amelioration-programs-for-the-community-quarantine-by-reason-of-covid-19\/\">List of Social Amelioration Programs<\/a>) implemented pursuant to the <a href=\"https:\/\/pnl-law.com\/blog\/bayanihan-to-heal-as-one-act-republic-act-no-11469-full-text\/\">Bayanihan to Heal as One Act<\/a> (<a href=\"https:\/\/pnl-law.com\/blog\/bayanihan-to-heal-as-one-act-republic-act-no-11469-full-text\/\">Republic Act No. 11469<\/a>) and its <a href=\"https:\/\/pnl-law.com\/blog\/implementing-the-bayanihan-to-heal-as-one-act-republic-act-no-11469-full-text-of-the-memorandum-from-the-executive-secretary\/\">Implementing Directives<\/a>:\u00a0<\/p>\n\n\n\n<!--more-->\n\n\n\n<p><strong>A. Senior Citizens<\/strong> &#8211; Senior Citizen\u2019s ID&nbsp;<\/p>\n\n\n\n<p><strong>B. Persons With Disability<\/strong> &#8211; Persons with disability ID; or Certificate of Separation from or Suspension of Work&nbsp;<\/p>\n\n\n\n<p><strong>C. Pregnant Women\/Lactating Women<\/strong> &#8211; Valid ID; Certification from RHU; or Birth Certificate of Child, if available&nbsp;<\/p>\n\n\n\n<p><strong>D. Solo Parents<\/strong> &#8211; Solo Parent\u2019s ID; or Certificate of Employment\/ Separation from or Suspension of&nbsp; Work&nbsp;<\/p>\n\n\n\n<p><strong>E. Overseas Filipinos in Distress<\/strong> (<a href=\"https:\/\/pnl-law.com\/blog\/target-beneficiaries-of-the-government-social-amelioration-program-for-covid-19\/\">definition<\/a>) &#8211; Valid Passport Bio Page; <strong>and<\/strong> Copy of any of the following:&nbsp;<\/p>\n\n\n\n<ul class=\"wp-block-list\"><li>1. Passport arrival stamp;&nbsp;<\/li><li>2. Proof of departure ticket;&nbsp;<\/li><li>3. Overseas Employment Certificate;&nbsp;<\/li><li>4. Employment Contract; or&nbsp;<\/li><li>5. Any other documents to prove that they were repatriated or banned from traveling outside the Philippines within the prescribed period.&nbsp;<\/li><\/ul>\n\n\n\n<p><strong>F. Underprivileged Sector and Homeless Families, Indigent Indigenous Peoples, and Other Vulnerable Groups<\/strong> (<a href=\"https:\/\/pnl-law.com\/blog\/target-beneficiaries-of-the-government-social-amelioration-program-for-covid-19\/\">definition<\/a>) &#8211; As applicable:&nbsp;<\/p>\n\n\n\n<ul class=\"wp-block-list\"><li>Certification from Barangay (proving that they are homeless families or belonging to other vulnerable groups) or from the CSWDO\/ MSWDO\/PSWDO where they are currently located.&nbsp;<\/li><\/ul>\n\n\n\n<ul class=\"wp-block-list\"><li>Certification from the NCIP or the tribal chieftain\/council of elders that the beneficiaries are Indigenous Peoples (IPs) and are on&nbsp;subsistence economy.<\/li><\/ul>\n\n\n\n<p><strong>G. Workers in the Informal Sector\/ Self-Employed<\/strong> (<a href=\"https:\/\/pnl-law.com\/blog\/target-beneficiaries-of-the-government-social-amelioration-program-for-covid-19\/\">definition<\/a>) &#8211; As applicable:&nbsp;<\/p>\n\n\n\n<ul class=\"wp-block-list\"><li>Driver\u2019s License;&nbsp;<\/li><\/ul>\n\n\n\n<ul class=\"wp-block-list\"><li>TODA ID; or&nbsp;Certification of membership from the Organization\/ Association; or&nbsp;Certificate of Public Convenience (CPC); or Certification from the&nbsp;Barangay\/ Municipality, for TODA only.&nbsp;<\/li><\/ul>\n\n\n\n<ul class=\"wp-block-list\"><li>Employment ID; or&nbsp;Kasambahay ID; or&nbsp;Certificate of Employment\/Separation from or Suspension of Work from the employer;&nbsp;<\/li><\/ul>\n\n\n\n<ul class=\"wp-block-list\"><li>Proof of business;&nbsp;<\/li><\/ul>\n\n\n\n<ul class=\"wp-block-list\"><li>Any other document showing employment\/ occupation\/enterprise; or&nbsp;Barangay Clearance<\/li><\/ul>\n\n\n\n<p><strong>H. Employers<\/strong> &#8211; Establishment Report; and Company Payroll<\/p>\n\n\n\n<p><strong>I. Rice Farmers<\/strong> &#8211;<\/p>\n\n\n\n<ul class=\"wp-block-list\"><li>RSBSA Enrollment Stub (for Cash Assistance)&nbsp;<\/li><li>Notice of Cash Grant (for Cash Assistance&nbsp;<\/li><li>Loan Application (for SURE Aid)&nbsp;<\/li><li>Any one (1) government-issued ID with picture&nbsp;<\/li><li>Duly accomplished simplified Promissory Note (PN)<\/li><\/ul>\n\n\n\n<p><strong>J.
