The Form (3 pages) is enclosed below (right click to download and save).]<\/p>\n\n\n\n<!--more-->\n\n\n\n<hr class=\"wp-block-separator\"\/>\n\n\n\n<h2 class=\"has-text-align-center wp-block-heading\">LABOR ADVISORY NO. 17-A<\/h2>\n\n\n\n<h5 class=\"has-text-align-center wp-block-heading\">Series of 2020<\/h5>\n\n\n\n<h2 class=\"has-text-align-center wp-block-heading\">ESTABLISHMENT REPORT FORM<\/h2>\n\n\n\n<p>Pursuant to Section 17 of <a href=\"https:\/\/pnl-law.com\/blog\/labor-advisory-no-17-guidelines-on-employment-preservation-upon-the-resumption-of-business-operation\/\">Labor Advisory No. 17<\/a>, Series of 2020, or the Guidelines on Employment Preservation upon the Resumption of Business Operation, employers are hereby advised of the&nbsp;<strong>RKS Form 5 of 2020<\/strong>. This Form shall be accompanied by establishments for: a) <a href=\"https:\/\/pnl-law.com\/blog\/flexible-work-arrangements-for-companies-due-to-the-coronavirus-disease-2019-covid-19\/\">Flexible Work Arrangement<\/a> \/ <a href=\"https:\/\/pnl-law.com\/blog\/employment-preservation-alternative-work-schemes-flexible-work-arrangements-wage-and-benefits-on-resumption-of-business-operations\/\">Alternative Work Scheme<\/a>; b) Temporary Closure; c) <a href=\"https:\/\/pnl-law.com\/blog\/retrenchment-or-downsizing-authorized-cause-in-employment-termination\/\">Retrenchment<\/a> \/ Reduction of Workforce; and d) <a href=\"https:\/\/pnl-law.com\/blog\/closure-or-cessation-of-operation-authorized-cause-in-employment-termination\/\">Permanent Closure<\/a>.<\/p>\n\n\n\n<p>The said report shall be submitted personally or online to the appropriate DOLE Provincial \/ Field Office which has jurisdiction over the workplace, effective 16 June 2020.&nbsp;<\/p>\n\n\n\n<p>Attached is a copy of the Form.&nbsp;<\/p>\n\n\n\n<p>Be guided accordingly.&nbsp;<\/p>\n\n\n\n<p>SILVESTRE H.
